Thank you for sharing how nurses can get involved in policymaking and the challenges we may face. I agree that getting involved in a professional organization is a great way for all nurses to become involved in policy. I will personally most likely end up joining at least two professional organizations. I would like to become a member of the American Nurses Association (ANA) and the American Psychiatric Nurses Association (APNA). The ANA is beneficial at a federal and state level by lobbying, representing nurses in federal agencies, and advocating at the state level (American Nurses Association. n.d.). Because my specialty is in psychiatric nursing, I will also join the American Psychiatric Nurses Association (APNA). As a member of the APNA, there is access to continuing education, updated best practices, networking with other professionals, a center for resources, and much more (APNA. n.d.).

Getting involved in policy making decisions at the organizational level is a great suggestion. This would be especially helpful for a nurse who plans to continue at their organization for a long time. I have been at my current employer for many years, and intend to stay at this organization once becoming and APRN. Thankfully there is strong leadership at my organization and becoming involved in committees is highly encouraged, and even rewarded. This will not have much impact on a large scale, but is a great way to improve and advance your practice where you work. Thank you for another great week of discussions!

Challenges Within Policy-Making

As we have discussed before, nurses have the ability to get involved with politics and policy-making by joining a professional nursing organization such as American Nurses Association (ANA) or American Association for Critical Care Nurses (AACN) for RNs and American Association of Nurse Practitioners for APRNs and so many others for every specialty within the nursing profession. RNs and APRNs also have the opportunity to run for office, whether it is for the local board, state legislation or being present at a federal level. “Nurses’ policy influence is nurses’ ability in the influencing decisions and affairs related to health through political knowledge, effective communication, and collaboration with other members of the health team, which results in the improvement of nurses’ job environment and increases patient outcomes” (Arabi et al., 2014).

With presenting new policies there will always be challenges needing to be overcome in order to implement the proposition at hand. One of those challenges seem to be within the first step of introducing the bill to the House of Representatives and Senate for it to be passed on to the appropriate committee. In order to avoid this challenge, getting involved with lobbyists and constituents to obtain endorsements from legislators, influential community leaders and professional associations allows the presenting individuals to go in with support of those who are more familiar with congress (Milstead & Short, 2019). Another challenge may be in the verbiage of the presented policy, which is why it is important that it is “drafted into the proper legislative language” (Milstead & Short, 2019).

Nurses at all levels deserve to know that their voice matters, and their voice can make a difference. The importance of ANA relative to policies needs to be expressed more intensely so nurses can see the hard work being done “behind the scenes” when it comes to policies. “ANA believes that advocacy is a pillar of nursing. Nurses instinctively advocate for their patients, in their workplaces, and in their communities; but legislative and political advocacy is no less important to advancing the profession and patient care” (ANA, n.d.). Another strategy I would implement would be starting a policy making team or suggestion box within my hospital organization so that those who have concerns or suggestions to better the work environment and patient care can truly feel like the propositions they are presenting are being considered instead of feeling like they are falling on the deaf ears of administration. Nurses need to feel that they are appreciated and in order for that to happen, changes need to be made to improve the issues they are presenting.

Two Opportunities for Nurses to Participate in Policy-making

There are a number of ways for a nurse to become involved in politics and policy-making. Two of these ways to get involved are by joining a professional nursing organization and to get involved at the state level. It is important for nurses to participate in policy-making, as many policies directly affect the nurse’s scope of practice and evidence-based care. Milstead & Short (2019) explain how nurse leaders are becoming more involved in this process.

One way for a nurse to become involved in policy-making is to join a professional nursing organization. Morin (2021) highlights how professional nurse organizations “play a critical role influencing policy by having their representatives provide testimony on health-related issues to national government leaders.” One specialized nursing organization that is relevant to my own practice is the American Psychiatric Nurses Association (APNA). According to Tierney (2021), as nurses, our “strengths and abilities of our profession empower us to steward health care transformation and revolutionize access to care.” The APNA is a well trusted organization that has the ability to influence policy-making decisions. Becoming a member of most nursing organizations is as simple as logging on and signing up. This will often include annual dues. A barrier may be the cost to join the organization, but the benefits of being a member are worth the cost and contribution. Joining a professional nursing organization like the APNA is a great way for nurses to get involved in policy.

Nurses can get involved in policy-making at the state level to help influence regulations that pertain to their interests and needs as a practitioner. It was found that many nurses do not participate in policy-making because of insufficient resources and policy-making knowledge (Hajizadeh et al., 2021). Many policies that directly effect a nurse’s scope of practice and abilities to practice are made at the state level. It can be intimidating for a nurse to try and get involved at a state level, it is important for institutions to provide information and resources. For instance, an APRN will have different abilities for independent practice in different states. This is why it is vital for nurses to become involved in policy-making at a state level.

Nurses are one of the largest contributors to healthcare in any nation.  From care to achieving patient positive outcome, nurses play one of the greatest significant roles.  Despite the fact that nurses are one of the largest contributors to healthcare in the nations and also large in number, nurses are supposed to be one of the highest policy stakeholders in the health-care industry.  However, the influence of nurses on policy is not proportional to the numbers (Perry & Emory, 2017). 

Two Opportunities for Nurses to Participate in Policy-making

One major opportunity that is open to facilitate active participation in policy making is for nurses to belong to professional organizations.  Nursing professional organizations are a mouth for nurses.  They are able to use their numbers to advocate, lobby and present issues of interest to legislative bodies across the nation.  Partnerships among nurse leaders in professional organizations, academia, and practice settings, aimed at educating nurses in policy engagement provide opportunities for RNs and APRNs are another way of enhancing nurses’ policy participation (Perry & Emory, 2017).   

According to Hahn (2019), “Individual APRNs have the opportunity to volunteer and be appointed to their own state’s regulatory committees to contribute to the promulgation of APRN regulations current and future”.

Challenges

One of the challenges is that because there are different nursing organizations with representation in different states there may be a lack of alignment and collective professional direction between nursing groups in some states.  However, in some states the APRN nursing collaborative processes are intact, benefitting from the formation of coalitions (Hahn, 2019).

Ways to Overcome the Challenges

One way to overcome challenges is the strategy of communicating and working together for a common goal. The interprofessional barriers that cause organizations to pull apart when they do not share common goals or possess an understanding of each other’s practice roles could be minimized through effective communication. According to Hahn, 2019, engaging lobbyists to represent APRNs in legislatures and involving APRNs as a voice to policymakers is a key to successful participation.

Two Strategies to Advocate for these Opportunities

Enlisting the advice of political lobbyists for guidance through the legislative environment working with leadership expertise, legal consultation and topical documents is a main strategy in addition to being in tune and in touch with the political ambiences in our jurisdiction, listening to respectable trustworthy lobbyists who possess the time and energy required in moving forward with a particular advocacy.

Another strategy is to identify what level of government is appropriate to engage (ANA, n.d).  Therefore, when there is confidence that the bill may pass in one branch of the legislature but not in the other, to save those resources for a legislative session where there is higher probability that there is a realistic opportunity to be successful in both chambers and to try to get something enacted into law (Perry & Emory, 2017).

Nursing organizations also endorse congressional candidates that demonstrate exemplary support of nurses and will continue to champion the profession in Congress and promote laws that positively influence health policies (ANA, n.d).

Of critical importance, healthcare professionals are the main implementers of policies and hence their inclusion in the formulation process becomes critical. This is because the nursing workforce is responsible for the direct care of patients. Therefore, nurses must be engaged in each of the phases of the nursing process (CDC.gov, 2022). Advanced nurses should contribute information regarding how the proposed policy will operate (CDC.gov, 2022). They should also suggest an appropriate strategy for engaging the relevant policy actors and stakeholders in the policy process. Registered nurses should also be involved in drafting the policy to address healthcare issues. According to Hajizadeh et al. (2021), there must be a motivation for nurses to participate in health policy-making processes since registered nurses have influence through their experiences on policies, laws, and regulations that govern the healthcare system. Registered nurses have the skills and courage, along with public respect for their work, to be involved and influential in health policy development. Nurses experience the daily result of policy and politics in health care. They are the largest member of the healthcare workforce and as such can advocate and influence for change. There are numerous ways for nurses to participate in policymaking.

One way is to participate in State and national nurses’ association- the American Nurses Association, the American Academy of Nursing, to name a few. Nursing associations keep a list of members that can write letters and make phone calls to their state and federal representative (Morris, 2023). With the nurse’s personal experience on how policies were implemented on the floor, they can give quality feedback to policymakers or provide useful data for research. Another way is to participate in evidence-based data. Nurses can initiate data-gathering, analyze data, and interpret research for their state and house representatives. They can also develop current research and analyze them in layman’s term and provide recommendations for policy (Morris, 2023).

Unfortunately, there are many barriers that hold back a nurse from participating in policymaking. Family and work demand would be top of the list. Most nurses, after working long hours would prefer to spend their time with family or personal care. Gender-based roles are a big factor as well. Having majority of nurses as women, most assume a more traditional role after work such as childcare or taking care for elderly parents (Anders, 2021). Another barrier would be the nurse’s lack of knowledge of the political process. Their limited understanding about elected officials and their stand on certain health policy.

Nurses are subject matter experts, having specific academic knowledge and clinical experience, nurses are a resource. Enhancing the nurse’s background and professional values, empowering them to not only be a tool but also an influence for the people around them, being the voice of not only their patients but for their peers as well; promote an environment of ownership and partnership. Knowing their worth in the community encourages them to be more involved- that as an individual they can influence change.

To help foster participation in policymaking, I would encourage nurses to further their studies. Enroll in a Master’s program on their respective field, or have a sub-specialty. Expanding once outlook allows you to access more resources and expand your influence. Being a wound specialist for example allows you to access more up and coming studies and equipment,you would be in aposition to give recommendations  to doctors and administrators. Another way is to promote advocacy. Tapping on the nurse’s role as advocates and being able to see the health systems’ effectiveness or lack thereof firsthand, is the perfect mouthpiece, an expert that can interpret for policymakers. Being a member of your local nurse association and being able to voice concerns on nurse’s work conditions, raise in wages, or organize symposiums for community outreach.
