Veterans’ healthcare is an issue that has reached the presidential level, and the current and former presidents have signed laws to improve healthcare access and mental health services for this population (Hester, 2017). Improving access and quality of healthcare to veterans is important because this population experiences substance use disorders, mental health disorders, post-traumatic stress, chronic pain, amputations, and traumatic brain injury at disproportionate rates compared to their non-veteran counterparts (Oster et al., 2017). This week’s discussion will discuss the issues in America that are serious enough to be addressed at the presidential level. As I think about the healthcare field & the many issues and concerns, mental health and the opioid crisis stand out the most. Mental Health illnesses continue to affect millions of people across the world. When a person is mentally unstable, that person experiences dysfunction, disorientation, emotionally unstable, and experience high-stress levels. There is a higher risk of death rates among people with mental illnesses. I noticed that the lack of access to healthcare and the need for more mental health clinics dramatically contributes to the ongoing mental health crisis.

On April 29, 2002, in a speech in Albuquerque, President George W. Bush announced the President’s New Freedom Commission on Mental Health, the first presidential mental health commission in 25 years(Hogan, 2003). Bush also stated, “We need a health care system which treats mental illness with the same urgency as physical illness” (Hogan, 2003). Bush discussed the barriers that affected mentally ill people and their healthcare. The stigma remained the same by the opposing viewpoint on mental health, high unemployment, and disability for adults with severe mental illness; mental health and suicide prevention are not a national priority, and fragmentation and gaps in care for adults with serious mental illness(Hogan, 2003)

Bush established the NewFreedom Commission for mental health to address, bring awareness and acknowledge the concerns and issues regarding care and treatment for individuals with disabilities and provide appropriate healthcare services. President Bush wanted New Freedom Commission to bring new ideas and requests to improve mental healthcare delivery in America. Bush established six goals to accomplish as he tried to transform the mental health system. The six goals were as follows: Americans understand that mental health is essential to overall health, mental health care is consumer and family-driven, disparities in mental health care are eliminated, early mental health screening, assessment, and referral to services are commonplace, excellent mental health care is delivered, and research is accelerated, and lastly technology is used to access mental health care and information (President’s New Freedom Commission on mental health, n.d.). These goals focused on removing the stigma attached to mental health illness and ensuring that all individuals battling with mental illness or caring for individuals dealing with mental illness received nondiscriminatory services and efficient and quality care.

The Affordable Care Act was established in March 2010 by President Obama. The ACA will eliminate out-of-pocket costs for recommended preventive services for adults and adolescents (National Archives and Records Administration, n.d.). The ACA aimed to form the mental health & substance use disorder parity task force to work together to ensure that Americans benefit from MH and Substance use disorder parity protections (National Archives and Records Administration, n.d.). ACA was a great benefit for the mental health community. It allowed private insurance & Medicaid coverage at attractive rates, which can be affordable for the mental health illness population. 

Through the expansion of comprehensive private health insurance and Medicaid, people have experienced improvements in their mental health; With the ACA, millions of people with mental illness are eligible for Medicaid, the nation’s largest payor of mental health services (NAMI, n.d). In 2014, ACA eliminated medical underwriting in the individual and small group markets starting in 2014, so medical history — including mental health history —no longer results in enrollment denials or higher premiums(Louise, 2020). President Obama signed the Clay Hunt Suicide Prevention for American Veterans Act in 2015. 

This act was enacted to ensure that veterans with mental illness received the appropriate, immediate, and effective care and treatment. Resident Obama declared May the official Mental health awareness month starting in May 2016. He wanted to bring more awareness to the population around the world at governmental agencies, companies, organizations, and healthcare professionals, acknowledging that it remains a crucial problem in our country. 

In 2019, President Donald Trump pledged to expand mental health programs. During his fiscal year 2019 budget, he took steps towards 8.6 billion to expand & transform VA’s focus on mental health services & increase funding for community, non-hospital treatment and mental health services for individuals involved with the criminal justice system, as well as maintain funding for community mental health services block grants (PolitiFact, n.d.). 

The mental health advocates praised these efforts from the trump administration but expressed concerns about other budget parts and the administration’s agenda. The budget reiterates the administration’s desire to repeal the Medicaid expansion enacted in some states under the Affordable Care Act, called Obamacare. That is a problem for this promise because “upwards of 40 percent of adults covered through Medicaid expansion are people with behavioral health conditions” (PolitiFact, n.d.)

In conclusion, being in the healthcare field and a healthcare provider, having access to non-discriminatory and affordable health insurance is imperative in America. Being a psych nurse for nine years, I see the constant battle of hopelessness, depression, and lack of self-worth and care in the mental health population. I have worked with many psych patients battling stress, anxiety, and depression because of the lack of mental health resources and healthcare services. If I were in the position, I would continue to go President Obama route by keeping the ACA in place, allowing affordable healthcare services for all people. I would strive for ongoing and continued coverage of mental health benefits and services rather than trying to eliminate access. I would continue to provide funding to SAMHSA (Substance Abuse &  Mental Health Services Administration), which allows for support and ongoing advocacy to the mental health population. Suppose the funding was able to be redistributed to SAMHSA. In that case, it can allow more financial support to mental health programs that can bring awareness, a significant impact on mental health, and a better outlook for the mental health community.
