Among the conditions that have been having ever-growing health concerns is chronic kidney disease. While a substantial number of people are living with the condition, several others are at risk of getting the disease, such as people living with hypertension and diabetes, among others. Besides, a notable percentage of people who live with chronic kidney disease usually have other comorbidities such as heart failure, hypertension, and diabetes (Donald et al., 2018).  it is therefore important to engage in strategic care plans that can prevent those who are at risk of the condition from getting it and slowing down the progression of the disease among those who already have it.  Nurses have a critical role to play in terms of education and health maintenance, among other interventions, to ensure that such programs are successful. Besides, the success of the interventions is checked by using various evaluation methods, supported by evaluation plan. This paper involves a discussion of evaluation methods to be used in monitoring the interventions used in slowing down the progression of chronic kidney disease among patients living with the condition. Besides, the paper will also discuss an evaluation plan.

Evaluation Methods

Evaluation methods are vital in monitoring the progress of an intervention. The evaluation method or strategy targeted to be used is a summative evaluation of the intervention for slowing the progression of the condition. The summative evaluation will come in two forms, outcome evaluation, and impact evaluation. While outcome evaluation deals with the short-term and medium-term anticipated changes resulting from the education intervention, the impact evaluations deal with long-term results as a witness in the whole patient population. The evaluation model which will guide this evaluation is the RE-AIM model, a model that has been comprehensively applied in measuring the impacts of various research activities done in real-world settings (Shen et al., 2020). Since it is based on the evaluation, four dimensions of the model will be considered including, effectiveness domain, the reach domain, which entails the number of patients with the condition reached out, the adoption domain, which entails the proportion of people who use the intervention, implementation which entails the completion of the intervention protocol.

Performance Measurement

The proposed interventions majorly aim at reducing the chances of the diseases progressing to risk or slowing its progression. Therefore, the interventions proposed including lifestyle modifications and blood pressure control. Such interventions have been shown to have a double benefit when it comes to reducing cardiovascular risk (Tu et al., 2017). So the performance of the intervention will be measure on how well the patients control their blood pressure. With adequate education provided about the importance of controlling blood pressure as lowering a raised blood pressure is a major factor in slowing the progression of the condition, the patients will be expected to display a controlled blood pressure. For instance, a diastolic pressure< 90 mmHg and systolic blood pressure of 120 to 139 mm Hg. Even though for patients having other comorbid conditions like diabetes, a different target will be monitored. While the short and intermediate health outcomes measured will be the blood pressure within the desired ranges, the long-term effects will the overall reduction of progression of the diseases among the identified group of patients.

Performance Monitoring

As indicated earlier, controlling blood pressure is key in ensuring the slow progression of chronic kidney disease. Therefore the performance of the educational intervention on self-management strategies and blood pressure control will be monitored by frequently checking the blood pressure of the patients, preferably two times a month, to ensure that they stay on course as a short and intermediate health outcome. Some of the education items or points offered to patients to ensure that they control their blood pressure include consumption of heart-healthy meals, eating low sodium meals, doing physical exercise to stay active, and quitting smoking. Besides, the glomerular filtration rate and albuminuria should be monitored at least once every twelve months as part of the long-term effect. Since the program is designed to go for 24 months, a summative evaluation will be done by the end of the period. The patients will be checked for the final readings of both systolic and diastolic blood pressure and compared against the expected standard levels.

Evaluation Plan

Successful implementation of an intervention requires that the program is evaluated as stated in the evaluation plan as the process is appropriate in determining how effective the intervention has been, therefore leading to refining the delivery of the program. Evaluation not only offers feedback on the intervention’s effectiveness but also assists in establishing how appropriate the intervention is to the targeted group of patients living with chronic kidney disease(Shlipak et al., 2021). The evaluation plan involves various stages. Even though there are many approaches to formulating an evaluation plan, a good evaluation plan should have six major components, including, description of the program engagement of stakeholders, focusing on the evaluation design, data collection, data analysis and interpretation and dissemination of results, and lessons learned.

Effective management of various diseases requires that a healthcare professional identifies those who are at risk and those who are living with the condition for the creation of an effective care and management intervention. However, the process does not end with implementation as the interventions need to be evaluated to get suitable feedbacks that can be used in improving the program. This paper has discussed an evaluation plan for intervention for patients living with chronic kidney disease.
