Patient safety in operating room is essential and starts before the patient gets into the operative suite and entails attention to all applicable types of preventable medical errors, especially surgical errors that are unique to this setting.  Surgical errors that include operating on wrong patient, wrong site, wrong procedure and retention of foreign objects affects the quality of care offered to patients (Abbas, 2020). Therefore, as a member of the operating room team in a hospital reporting a rise in complications rates, it is essential to establish the root cause of these complications and interventions that can be implemented to prevent and reduce their occurrences (Smith et al., 2019). Quality models are important in improving patient care quality and outcomes, especially the prevention of surgical complications like retaining foreign objects in the patient.

The quality improvement model that I would use in addressing the causes or adverse events is the root cause analysis (Buljac-Samardzic et al., 2020). The Root Cause Analysis (RCA) is a quality improvement tool deployed to define the primary cause of an issue and identify the necessary actions to permanently eliminate such occurrence. A more effective RCA process enables hospitals to establish safer and more trustworthy care environment for patients. Initially designed to analyze industrial accidents and events, the RCA is used widely as an error analysis tool for healthcare providers.

At the core of this model is to identify the underlying issues that increase the possibility errors while avoiding focusing on individuals for mistakes (Karkhanis et al., 2020). In this case, the model will entail a systematic approach to identify these errors; active errors and latent errors. The application will entail educating the team on how to apply the rout cause analysis through effective communication and a collaborative approach to catch the errors.

The addressing of adverse events in an operating room require focusing on using quality patient care services. A quality model provides a hospital with the appropriate way to increase the delivery of services to the different clients. Kaizen quality improvement model require making changes that will improve operations of a business.

The gradual improvement of processes is an approach to help in dealing with a challenge of quality facing an organization. Kaizen model involves using the right ideas to ensure that the work environment is more efficient and effective for increased performance in the industry (Abuzied, 2022). Kaizen model will ensure that the operating room work will become more fulfilling, less tiring and safer by addressing the different complications faced by the patients. Kaizen philosophy require focusing on offering standardized work that will deal with complications in the organization.

Kaizen model is essential in making sure that the different workers in the organization are contributing to improvement and quality performance.
