A problem that has been an issue within my workplace is the dialysis technicians not notifying the nurses when a patient’s blood pressure (B/P) has changed.  Since dialysis has such a fine line between removing fluid from the patient and having a stable B/P, the facility has policies and procedures in place to alert the nurse in situations when a B/P has had a change.  When a B/P decreases quickly or unexpectedly, a dialysis patient can become unresponsive, have seizure activity, have problems with breathing or in a worse-case scenario, pass away.  The technicians go through a three-month training period prior to being “on their own”.  In this training period, they learn normal vital signs and the vital signs that are out of range. 

Per our policy, nurses need to be notified of a decrease or increase in the B/P of 20 mmHG or more.  Our vital signs are done automatically by the dialysis machines every half an hour and it is policy for the technicians to do their “line checks” in a timely manner after this period (within 10 minutes).  When the line checks are completed, the computer will automatically time stamp the exact time the check is done.  When there is no notification to the nurse on changes in vital signs, the nurse does not have the knowledge to intervene and manage the patient prior to having an adverse event occur.  

To address this problem, first I looked into the job description for a dialysis technician.  A technician’s duty is to “monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. Please note you will have exposure to blood and needles” (Davita, 2023).  The job description was brought to the attention of the technicians and read during a homeroom meeting with the emphasis being placed on the monitoring of patients during and after a dialysis treatment.  After this step, I provided a short class on what a “reportable” vital sign is.  Any change in a systolic pressure of 20 points either increased or decreased must be reported to the nurse.  Also, a diastolic reading of <60 is reported.   Another important portion of the vital signs is the heart rate.  A heart rate <60, needs to be reported.  After the vital signs class, continuous monitoring of the technicians is required with ongoing support, teaching and education.  

I must admit, that with this process, only the first two steps have been achieved.  Educating staff is an ongoing process as situations change throughout the workday and situations with patients are not always “set in stone”.  My fellow nurses and I are continually working with the technicians to enforce this new education.

It is important to follow a process when there is an issue or problem as all solutions rely on some kind of a process to problem solve.  Most people have a process when solving problems in their personal or professional lives, but the process has become almost second nature.  “Problem solving is the act of defining a problem; determining the cause of the problem; identifying, prioritizing, and selecting alternatives for a solution; and implementing a solution” (American Society for Quality, 2023).  The problem solving process consists of four steps.  The first step would be to define the problem or issue that needs to be solved.  For example, If the problem or issue is in the workplace, the problem would need to be specific to determine whether a procedure or standard has been broken. 

The second step would be to generate alternative solutions.  In this step, the individuals that were involved with the initial issue would be involved.  This would lead to a brainstorming session on how to best solve the issue.  The third step would be to evaluate and select a solution.  For example, it would be best to select a solution that when implemented does not create additional issues that are related to the initial problem.  Finally, the last step is implementation.  Follow-up remains important to ensure the solution that was created is working (American Society for Quality, 2023).   If the original solution is not working, it is important to start the four-step process over again until the problem is resolved.
