Informatics and Data-Mining Evidence and Application

In this assignment, you will discuss informatics or data-mining evidence as it pertains to your topic. Consider whether your site is a Stage 1 or 2 meaningful use site. How does this impact clinical quality?

Consider the impact that data mining will have on future clinical quality. Everything we do in nursing should be supported by evidence. Title: NSG 7220 Week 6 Discussion: Informatics and Data-Mining Evidence and Application

In the area of Texas that I live in, most healthcare agencies have converted to electronic health record systems. However, I do know of some psychiatric and long-term acute care facilities that are still using paper charting instead of adopting meaningful use standards. Meaningful use was created to help improve healthcare services by incentivizing healthcare organizations and providers when implementing electronic health record systems (Charles & Del Vecchio, 2018). Implementing electronic health record systems helps improve the quality of care and increases positive health outcomes for clients by providing enhanced services to community members (Charles & Del Vecchio, 2018).

Stage 1 of meaningful use involved purchasing and implementing electronic health record systems, and stage 2 of meaningful use wanted the healthcare organizations and providers to enhance the operability and clinical processes within the electronic health record system (Charles & Del Vecchio, 2018). At my approved clinical site, examples of how they met stage 2 of meaningful use are that they started enhancing how the electronic health record system could increase patient satisfaction, increasing care coordination, and by moving the doctor orders from the paper chart into the computerized order entry section of the electronic health record system (A. D., personal communication, November 29, 2023). Stage three of meaningful use has been achieved at the practicum site also because they have continued to increase the interoperability of their clinical care processes by including several clinical decision support alerts to improve safety and offering patient health portals and advanced care coordination by continuing to improve and utilize the electronic health record systems.

Meaningful use and care standards are involved with the proposed project of implementing a de-escalation training course for staff to help patients in crisis. The clinical site may even incorporate the developed de-escalation tips within the electronic health record system in the documentation section to help nursing staff implement interventions and give examples of how they can de-escalate the situation or document what they have completed to help the client. A de-escalation tab could be added under interventions with examples of interventions the nurse could implement. This would allow the nurse to point and click to add the interventions they have been trying to complete to help the patient. Moreover, this would enable the nurse to improve documentation methods and improve patient care. The provided documentation can then be evaluated and used to collect data to enhance the de-escalation process, provide insights, and improve the effectiveness of the de-escalation strategies taught in the training program.

I had difficulty finding articles by only searching for informatics and my project topic. So, I made the search more specific to searching for telehealth and electronic healthcare-related articles instead since it gave me more options. The first article that I reviewed and was interested in was an article about an outpatient clinic that started providing telehealth psychiatric services. The study involved forty adults in Denmark who were offered the option to receive either telehealth services or clinical visits with the doctor in person (Shaker et al., 2023).

The first study only allowed clients seeking new services and those with “anxiety, depression, and personality disorders,” but they did not allow patients with addictions, schizophrenia, or bipolar disorders (Shaker et al., 2023, p. 2). I feel this was a limitation of the study because they only allowed certain types of individuals to have the option to receive mental health virtual care. The excluded individuals may also have benefited from the care options and helped avoid crises when they could have visited virtually with a mental health professional. Another limitation of the study was that not all of the initial forty recruits followed through and completed the study (Shaker et al., 2023). The treatment for the participants offered different types of therapy sessions, prescriptive services, and crisis services (Shaker et al., 2023). The results of the services (in-person and telehealth) both reported a decrease in acute mental health signs and symptoms, which was determined to be statistically significant since p < 0.05. Telehealth services should be increased in the inpatient setting when clients are in crisis, so this type of psychiatric telemedicine services could be implemented at the hospital. When clients are being taken care of medically, it is still important to care for their psychological problems, so increasing the instance of psychiatric telehealth services could be used to help maintain and decrease crises.

For the second article, the researchers reviewed files of individuals seeking virtual mental health services due to the option becoming available during the coronavirus pandemic (Sanchez-Guarnido et al., 2022). The records of clients seeking telehealth services were reviewed from several clinics in Spain before, during, and after the coronavirus pandemic (Sanchez-Guarnido et al., 2022). The clients who received the telehealth visits had a statistically significant (p < 0.05) decrease in the number of acute services that were necessary since they continued to receive their mental health services via telemedicine services (Sanchez-Guarnido et al., 2022). Telehealth services should be a part of everyday services to increase the chance of the client being able to seek services to maintain their mental health.

Both reviewed studies noted a decrease in crises or situations requiring hospitalization due to receiving telemedicine services, which is very beneficial to their mental health. Telehealth services could be incorporated into the project idea in different ways. The staff could also be prompted to contact the medical provider in the hospital to request a psychological referral to help increase optimal care services for clients in crisis. Increasing the ability of the clients in a medical hospital to receive psychiatric services is essential since we need to think of caring for the client as a holistic and coordinated effort between various healthcare team members.
