In this assignment, you will discuss in detail your topic of interest for your practicum. You will also be required to list at least two individual behavioral objectives (which include verbs that can be measured) at the beginning of the semester, on the basis of overall objectives of this course, as well as your topic of interest. My preceptor is currently a manager in a critical care area and has been an educator in higher education programs, acute care, and critical care within hospital systems. She has been a registered nurse for over twenty years in various fields, including acute care nursing, cardiology, critical care, and management in various hospital and clinical settings. She has earned her doctorate of practice in nursing practice from the University of Texas at Austin, and she also has a post-graduate certificate in nursing education. Her first graduate degree was in business administration, so she has extensive experience and will be a valuable part of my success in this doctorate program.

I will be completing my preceptorship at a major acute care hospital in Texas, and I will have access to acute care units, critical care units, and emergency services due to my connection with my preceptor and her position within the hospital as a manager. As a result, I will have several options for topics to choose for my practicum doctor of nursing (DNP) project. This week, I was able to connect with my preceptor to start discussing the project and brainstorm about the DNP project.

Honestly, I am unsure what I want to focus on for my DNP project. I originally wanted to focus on nursing education by developing tools for mentoring new adjunct faculty members to ensure they are set up for success. Mentoring is an invaluable service that will help guide the new faculty member through the transition from an expert clinician to an educator, and receiving mentoring services can also help curb nurse educator turnover to help retain faculty (Knowles, 2020). However, this topic was not something I was told could be approved since the project needed to occur at a practice site.

After speaking with my preceptor, we discussed the possibility of preventing skin injuries in either critical care or acute care at the clinical site. This is an interesting topic, but since I have been thinking of focusing on a mental health-related topic, my preceptor contacted the emergency services manager to schedule a meeting to discuss what identified safety or mental health concerns need priority attention in that department. I have yet to schedule the meeting with the emergency services manager, but I have a couple of ideas I was thinking about.

If I decide to focus on mental health-related interventions at the clinical site, I have thought about ways to improve de-escalation techniques during a crisis. A goal would be to help to increase the safety of the patient and the staff, as well as being able to decrease a crisis situation before it escalates. I want to investigate what the current de-escalation techniques are for emergency services and what the results have been concerning the existing techniques. I hope to also talk to emergency services nurses to see their experience when treating patients seeking help at the hospital for a psychiatric crisis.

Examples of possible behavioral objectives that could be used for this mental health topic are:

  1. Research and assess current de-escalation techniques and the training program available at the clinical site to help treat adult clients experiencing a mental health crisis.
  2. Evaluate evidence-based research pertaining to de-escalating crises for adults seeking emergency mental health services.

Since the mental health topic will be dealing with clients in crisis, it may be difficult to get their consent or be able to ask what they have experienced and be able to have clients that are competent to gather information from. I have not been able to visit the emergency services section of the site yet. However, when I do, I will interview the manager, staff nurses, and patient care technicians to identify concerns that they feel require priority interventions to improve safety and increase optimal health outcomes for the adults with mental health crises coming to the emergency room. Appropriate de-escalation techniques have been implemented before that decreased the use of restraints (Wong et al., 2022). I hope that I will be able to reduce and prevent restraint use. In my experience working in mental health nursing, restraints can cause more trauma and distress to the client. Gottschalk et al. (2023) noted that restraints can cause physical and emotional trauma. I still have a lot of research and conversations to complete before narrowing down my DNP project topic, but I am trying to stay optimistic.
