Nurses strive to provide the best possible care to diverse clients under consistently changing conditions. From the medications administered to the type of dressing used to heal a wound, nurses apply procedures that have been tested through research and deemed appropriate according to evidence-based standards of practice. Through foundational knowledge related to research methods, translation of research data is used to improve nursing practice and, ultimately, patient outcomes. Therefore, nurses must become familiar with the specific language of scientific research and the research process. As health care professionals, nurses seek to provide their patients with the best possible health care. To determine which approaches to care result in the best possible care, the effectiveness of each approach specific to a chosen population must be investigated. The pursuit of knowledge is the basis for research. Researchers seek to find answers to various scientific questions, but there are the boundaries associated with the pursuit of knowledge (Helbig, 2018).

I work in a pulmonary office. We use the latest research to provide care with diagnosing our patients with COPD or Asthma. Using the latest research and guidelines is important.

Applying evidence-based practice is very important when it comes to healthcare. Because it includes the usage of the latest technologies, which often happens in my workplace. These provide better service to patients and prevent or eliminate medical errors in patients by the documentation of health information. By using the latest technology in my workplace, you can enhance clinical care and also by giving information about new clinical care. When there are patients with different health problems, technologies make taking care of them and assessing them a lot easier. It additionally, helps. nurses in my workplace, including other healthcare workers, make the right decision in the critical situation of patients in the health care settings. Further down, technology helps our organization improve the quality of care through evidence-based practice to enhance professionalism in nursing and helps avoid many of us working avoid dangerous situations.

Any nurse who follows a code of conduct inpatient care should apply evidence-based practice into their life. Better outcomes through evidence-based practice could enhance a nursing profession or an organization.

I work in a long-term acute care hospital, and I use to work long-term care at a nursing home. These are two very different aspects of nursing but very important work, nurses have to be alert and vigilant when it comes to patients and advocate for patients. Research is needed to take care of the patients and make decisions about care; it helps with what a nurse plan on doing and what they can do. When I was working in the nursing home our biggest concerns were falls, pressure ulcers, weight loss relate to nutrition intake, EBP was/can be applied by the proven fact that turning patients every two hours and providing frequent incontinence care can help prevent skin breakdown/pressure ulcers. Research has shown that frequent rounding, toileting, and 5 Ps (potty, pain, possessions, etc) can help prevent falls. It shows that falls happen during shift change frequently, so my nursing home in forced bedside report and walking rounds,

(which help with multiply things for patients) we also used the falling star program (there was a picture of a falling star on the room door and on the patient wrist band, this way everybody knew the patient). These things being applied were the result of EBP and research from our quality control team at work. Working at the hospital LTACH we focused more on infections. EBP and research are used on patients with foleys and central lines for example preventing CAUTIs and CLASBIs. We do Chlorhexidine wipes on all new admits and on foleys twice a day, dressings for lines are changed weekly. Patients are assessed daily for both to see if it is needed to keep, this is due to research showing that having either on can lead to infection. I think nurses will always use EBP/research in the care they give if it’s Tylenol for s fever or sitting a patient up in bed to help them breathe.

In practically every sector of healthcare, nurses are starting to  utilize evidence to restructure their practice. These nurses are using something called “evidence-based” practice to go about the way they plan their nursing interventions, and the benefits of this practice are limitless. Briefly summarized, the benefit that evidence based practice (EBP) provides is that  “EBP enables nurses to evaluate research so they understand the risks or effectiveness of a diagnostic test or treatments” (EIU, 2018). This means that nurses can do research prior to diagnostic tests and treatments to understand what method is more beneficial for the patient. Since we live in a world where we have years of research preceding our practice this many not sound like a significant advancement, but with a broader historical glance it becomes evident how significant of an advancement EBP is.

Prior to the creation of nursing research and EBP, the nursing approach to treatment was more confined to the books (even if these treatments weren’t effective/not as effective as other methods). If a nurse strayed away from specific treatment plans then their experimental practices may cause significant harm to the patient, something that no nurse wants to occur. EBP helps us today because we don’t have to be confined to effective treatment, and we don’t have to risk experimental procedures when treating our patients. Today we are afforded the liberty of looking at previous nursing interventions and determining which one was effective enough to be incorporated into our daily practice. EBP is essentially utilizing an ever-expanding textbook of knowledge to fine tune and further our nursing interventions and it is something that the nursing profession will continue to benefit from for a very very long time.

I see EBP utilization everyday in my place of work. As a health care coordinator caring for geriatric patients, we tend to utilize EBP frequently to help prevent the occurrence of hospital acquired conditions such as pressure injuries . This includes things like constant turning of the patient to ensure a release of pressure on the areas where bony prominences are, the use of pressure relieving devices such as air mattresses, and proper nutritional intake to repair skin and uphold the skin’s integrity. Besides the prevention of these conditions, EBP also helps us treat them if they are to occur. I believe EBP will and should be essential to every aspect of the nursing field in the near future since it no only encourages good and efficient practice, but it furthers the amount of research conducted which will only lead to more advancements in EBP.

The healthcare systems focus on providing patient centered care that promotes the quality and safety of care. Research studies have been embraced in the nursing practice to help in unfolding new ideas and knowledge that help to bridge existing gaps in nursing. Research studies helps to identify and implement best evidence based practice in nursing. Evidence based practice in healthcare setting makes use of research outcomes that assist in the clinical decision making (Black et al., 2015). As a result, health facilities have to ensure that they adopt and implement best evidence based practices. It is therefore important for healthcare professionals to base their patient-centered care on research findings that are published in peer reviewed, systematic reviews and randomized control trials publications. According to Black et al.(2015) evidence-based practice is important because it aims to provide the most effective care that is available, with the aim of improving patient outcomes.

Patients are expected to receive the most effective care based on the best available evidence. In my facility, various evidence based practices have been embraced based on research findings from the hospital. A current incidence that have resulted in increased use of evidence-based practice at my facility is the increased cases of hospital-acquired infections. The healthcare setting has developed protocols and clinical guidelines to help in preventing infection spread. In an article by O’Donoghue et al. (2016), incidences of hospital acquired infections results in high mortality rates, increased morbidity and the length of hospital stay is also elevated. According to Luangasanatip et al. (2018). The healthcare facility in various department setting in the hospital have embraced increased education and training in the best techniques on hand washing to help reduce infection spread. Nurses are requested to educate patients to embrace a practice of ensuring regular hand washing with the recommended antiseptic solutions. The intervention really helped in reducing infection spread at the facility. The surgical teams, wound care nurses have also been educated on best practices to ensure disinfecting tools and equipment’s as well as ensuring utilization of protective clothes. It is therefore critical to entire appropriate use of evidence-based practices to help prevent and reduce incidences of adverse patient outcomes.

Nursing profession has always incorporated evidence based knowledge into practice and as nursing evolved into a more scientific discipline over decades, the application of EBP into practice has become an expectation or necessity in providing the highest quality of care to patients. By definition , evidence based practice or EBP “is the conscientious and judicious use of current best evidence in conjunction with clinical expertise and patient values to guide health care decisions.” (Titler MG 2008). In this class we learned that the best evidence is collected from randomized controlled trails, qualitative and quantitative research, case studies and expert opinions.

When the research provides new information, it has to be incorporate to practice by making the necessary changes to improve the quality of care. While some practice guidelines are fully based on high quality research data, others may be purely based on expert opinion if sufficient evidence is not available. Many hospitals have evidence based committees that review the clinical practice policies and procedures regularly to evaluate that these policies are aligned with the current research evidence. In the health organization that I work, we have a EBP committee that work collaboratively with the multidisciplinary team and quality department to implement new EBPs in the hospitals. One of the biggest EBP driven QI project that have been successful in producing good quality outcome of the Nurse driven CAUTI prevention bundle.

CDC recommends following strict insertion criteria for urinary catheters to reduce infection as research shows that minimizing the use and duration of catheter in all patients especially in the elderly reduced their mortality due to catheterization associated infections (CDC.n.d). Based on the hospital protocol, a nurse and MD has to review the criteria and complete the checklist before proceeding with the catheterization. RNs need to evaluate the need to continue the catheter on a daily basis and if does not meet the criteria, need to discontinue the catheter. CAUTI prevention bundle also guide the nurse on the daily catheter care and the required patient education. When a CAUTI occurs, a root cause analysis is conducted to determine the cause of the infection and the RCA follows with a action plan that is directed toward preventing future infection. These positive CAUTI cases are reviewed with the staff and this process provide great opportunity for the nurses to fully understand the value of EBP in clinical practice and health care decision making process.

Evidence-Based Practice (EBP) is the incorporation of “clinical expertise, most up-to-date research, and patient’s preferences to formulate and implement best practices for patient care” (Schmidt, 2018). Nursing practice has evolved overtime, many archaic practices are no more valid, as the role of evidence based practice in “supporting nursing care and contribute positively to patient outcomes across a variety of settings and geographic locations cannot be over stressed” (Profetto-McGrath, 2005). Research evidences has greatly expanded over years in nursing, medicine and other health care disciplines. This is necessary to meet the society and public expectations of health care systems in delivering high quality patient focused health care. EBP is indispensable to nursing, as it forms the basis of nursing care in the achievement of personalized and holistic patients’ care and provides opportunity for individualized, effective and dynamic nursing care [and this enhances efficient] clinical judgment which invariable encourages better patients’ outcome.

I work in long term acute care facility where most of our patients are geriatrics, vent dependent and at times unconscious. On few occasions the doctor order Foley catheter for patients that does not really need it, but nurses have been able to advocate for such patients and condemn the order incorporating evidence based reasons why inappropriate urinary catheter placement can be harmful to the patient. Also, for patient that catheterization is necessary, nurses always ensure thorough assessment for the right indications (such as urologic surgery, neurogenic bladder, etc.) and sterile precautions duly observed during insertion and daily maintenance to prevent catheter associated urinary tract infection that studies has confirmed to be the “most prevalent healthcare-associated infection worldwide” (Mavin & Mill, 2015).
